John Q. Archibald (Denzel Washington) is an impoverished factory worker. In order to support the family, the relationship between the two couples is increasingly indifferent and the marriage is on the verge of collapse. To make matters worse, misfortune befell their 9-year-old son, Michael.Michael collapsed on a baseball field one day, and when John Q. took his son to the hospital, he learned that his son had a severe congenital heart disease and needed a heart transplant that was far more expensive than his insurance would cover. After several unsuccessful attempts at communication, the hospital director decided to remove them from the transplant waiting list. This infuriated John Q., the father. He stormed into the hospital's emergency room with a gun and kidnapped innocent medical staff and patients as hostages, demanding immediate surgery for his son. This human battle against morality and law is not as simple as it seems.
